This National Advance Care Planning Week (16-22 March), take action to ensure your care will reflect your values and choices, no matter what the future holds. Suitable for people of any age or level of health, Advanced Care Planning gives peace of mind and spares loved ones from making difficult decisions without guidance. 

Don’t wait for a crisis. Join this free introduction workshop to learn what’s involved, how to get started and how to talk about your wishes with loved ones and health professionals. From simple steps to formal documents, you’ll be armed with the knowledge to start planning at your own pace. 

Hosted by the City of Rockingham and facilitated by Palliative Care WA, you’ll be provided with all the resources you need. Attendees will also be offered further free opportunities for support to put what you learn into action.
